Job Description:
Join our team as a dedicated Hospice RN Case Manager, where you
will be instrumental in delivering compassionate care to
individuals in the final stages of life. In this role, you will
work collaboratively within the hospice team to oversee and
organize the comprehensive care plans for patients and their loved
ones. Your main objective is to ensure the holistic needs -
physical, emotional, and spiritual - of patients are addressed
while upholding their comfort and dignity throughout this critical
phase of life.
Responsibilities:
Patient Assessment and Care Planning:
--- Conduct comprehensive assessments of patient's medical
conditions, symptoms, and pain management needs.
--- Develop individualized care plans in collaboration with the
interdisciplinary hospice team and the patient's family.
--- Regularly review and update care plans to address changing
needs and ensure the highest quality of care.
Nursing Care and Treatment:
--- Administer skilled nursing care to patients, including wound
care, medication management, and symptom control.
--- Monitor patients' vital signs, reactions to treatments, and
overall well-being.
--- Collaborate with other healthcare professionals, such as
physicians and palliative care specialists, to enhance patient
comfort and pain management.
Emotional and Spiritual Support:
--- Provide emotional support and counseling to patients and their
families during the end-of-life process.
--- Respect and accommodate patients' spiritual beliefs and
cultural practices, promoting a sense of peace and acceptance.
Coordination of Services:
--- Facilitate regular interdisciplinary team meetings to discuss
patient care plans and address any issues or concerns.
--- Coordinate the services provided by other members of the
hospice team, including social workers, chaplains, and home health
aides.
--- Liaise with external healthcare providers and community
resources to ensure comprehensive support for patients and their
families.
Patient and Family Education:
--- Educate patients and their families about the hospice
philosophy, the end-of-life process, and available resources.
--- Provide guidance on pain management techniques and strategies
for coping with emotional challenges.
Documentation and Compliance:
--- Maintain accurate and up-to-date patient records, documenting
all care provided and communicating with patients and families.
--- Ensure compliance with all relevant regulations, policies, and
procedures related to hospice care.
Bereavement Support:
--- Provide support to the family after the patient's passing,
assisting with bereavement counseling and resources.
Qualifications:
--- Must hold an active Registered Nurse (RN) license in the state
of practice in New Brunswick, NJ, US.
--- Should have a minimum of 2-3 years of clinical nursing
experience, with a preference for hospice, palliative care, or
related fields.
--- A deep understanding of hospice philosophy and principles of
end-of-life care is essential.
--- Excellent communication and interpersonal skills are required
to interact effectively with patients, families, and the
interdisciplinary team.
--- Demonstrates empathy, compassion, and sensitivity when
supporting patients and families through grief and loss.
--- Ability to work independently and collaboratively within a
team, showcasing strong organizational and time-management
abilities.
--- Proficient in using electronic health records and other
relevant healthcare software applications.
