About the RoleWe are seeking a qualified Registered Nurse to provide exceptional Behavioral Health and other community-based healthcare services in Hartford and surrounding areas. This is a Full-Time opportunity with flexible scheduling, including on-call requirements every 8 weeks.Responsibilities

  • A supportive environment with employee-focused wellness and support programsJob SummaryThe Registered Nurse in Home and Community Based Services will provide and direct the provisions of nursing and personal care to patients in their homes, as prescribed by the physician or authorized by the state/program, in compliance with applicable laws, regulations, and agency policies.Specific Job Duties/Responsibilities
    • Deliver high-quality clinical services within the scope of practice and infection control standards
    • Conduct clinical nursing assessments in accordance with federal and/or state program requirements
    • Ensure patient eligibility and medical necessity for services
    • Develop and revise individualized plans of care and/or service plansRequirements
      • A minimum of one year experience as an RN is preferred
      • Current RN licensure in the state of practice
      • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
      • Current CPR certification
